According to the Natural Medicine Division’s Code of Colorado Regulations, Rule 5005 (C)(2)(a), all Facilitator requests for transfer of regulated natural medicine must include the information on the form below. We will contact you via phone or email to verify your request. We accept payment in cash or bank/wire transfer at this time.
